Making a referral
Please use the general email address or telephone number on our Contact page to begin. Do not send protected health information by ordinary email.
Helpful information for an initial conversation
- Patient age and Massachusetts location
- General reason for referral, without sensitive clinical detail
- Referrer name, role, and contact information
- Whether the family is aware of the referral
Clinical records should only be exchanged after the practice provides an appropriate secure method and obtains any necessary authorization.
